Instructions
This is an assessment to be completed one-on-one with a teacher. The teacher will read all instructions and words aloud to the student.
Part 1: Letter Sounds
Teacher: "I am going to say a sound. Circle the letter that makes that sound."
Part 2: Beginning Sounds
Teacher: "I am going to say a word. Circle the letter that the word starts with."
Part 3: Rhyming Words
Teacher: "Listen to the first word. Then I will read three more words. Circle the word that rhymes with the first word."
Part 4: CVC Words - Vowel Sounds
Teacher: "I will say a word. Write the missing middle letter in the blank space."
Part 5: Reading CVC Words
Teacher: "Point to each word and read it out loud." (Teacher can make a checkmark next to correctly read words).
